Person-Centred Support

Promoting Independence
Support in the Community
Mental health challenges can affect every aspect of a person’s life. But support should never focus solely on symptoms. It should focus on helping people build confidence, develop resilience, and live a life that feels meaningful to them.
At Tao Complex Care, we provide specialist mental health support as part of our wider complex care services. Our approach is person-centred, flexible, and designed around the individual, recognising that no two experiences of mental health are ever the same.
Whether someone is living with anxiety, depression, trauma, emotional distress, or more complex mental health needs, we provide compassionate support that helps individuals navigate daily life while maintaining as much independence as possible.
Our role is to provide practical, emotional, and psychological support that empowers people to manage their wellbeing and achieve their personal goals.
Support may include:
Where appropriate, we can also support individuals who have physical disabilities or additional complex care needs alongside their mental health condition.
We believe people are more than a diagnosis, more than a risk assessment, and more than a care plan. That’s why every support package begins with understanding the individual, their history, their strengths, their aspirations, and the challenges they face.
Our care plans are developed collaboratively and reviewed regularly to ensure support continues to reflect changing needs, circumstances, and goals. By focusing on the whole person, we help create opportunities for growth, independence, and improved quality of life.
Meaningful support starts with meaningful relationships.
For many individuals living with complex mental health needs, consistency is essential. Trust, familiarity, and stability can play a significant role in wellbeing.
We carefully match support workers to individuals and focus on creating strong, positive relationships built on respect, understanding, and open communication.
Our teams adapt their approach to suit each person’s communication style, emotional needs, and level of understanding, creating supportive environments where people feel safe, valued, and heard.
Mental health support requires a careful balance of compassion, awareness, and proactive risk management. Our teams consider each individual’s diagnosis, personal history, identified triggers, and any associated risks when delivering support.
This includes awareness and management of:
Clear crisis management and escalation protocols ensure concerns are identified early and responded to appropriately, helping individuals access the right support at the right time.
We recognise that effective mental health support often involves multiple professionals working together.
Our teams work closely with:
By maintaining strong communication and collaborative partnerships, we help ensure individuals receive coordinated, consistent, and effective support.
Everything we do is designed to empower people, not create dependency. We support individuals to develop the skills, confidence, and resilience needed to take greater control of their lives, while knowing support is there when they need it.
This may involve accessing education, employment opportunities, social activities, community groups, healthcare services, or simply rebuilding confidence in everyday life.
Because good mental health support isn’t about doing things for people. It’s about helping people discover what they’re capable of achieving for themselves.
Our approach combines clinical awareness with genuine compassion.
We deliver mental health support that is:
Most importantly, we focus on possibility.
Because everyone deserves the opportunity to live a life without limits.
What mental health conditions do you support?
We support individuals living with a wide range of mental health conditions and emotional wellbeing challenges, including anxiety disorders, depression, trauma-related conditions, personality disorders, behavioural distress, and other complex mental health needs. Every support package is tailored to the individual rather than the diagnosis.
Is your mental health support part of your complex care services?
Yes. Mental health support forms part of our wider complex care offering. We provide personalised support for individuals whose mental health needs impact their daily life, independence, or overall wellbeing.
Do you work with Community Mental Health Teams (CMHT)?
Absolutely. We regularly work alongside Community Mental Health Teams, GPs, psychologists, psychiatrists, social workers, and other healthcare professionals to ensure joined-up, person-centred support.
Can you support people with both mental health needs and physical disabilities?
Yes. We understand that many individuals have multiple support needs. Where required, we can provide support for people living with both mental health challenges and physical disabilities as part of a comprehensive care package.
How do you manage risk and crisis situations?
We have clear risk management, safeguarding, crisis planning, and escalation procedures in place. Our teams are trained to recognise changes in wellbeing, respond appropriately to concerns, and escalate issues to relevant professionals when required.
How does Tao promote independence?
Everything we do is focused on empowering individuals to build confidence, develop coping strategies, strengthen resilience, and access opportunities within their community. Our goal is to support people to achieve greater independence while maintaining the right level of support for their needs.
